Add txout index to bitcoind json output #1613

If a transaction lists more than one output going to the same address, there is no way to distinguish those outputs in the bitcoind json output. This issue is to propose that txout index be added to the data listed for each transaction output shown by listtransactions.

I really need this as well. @laanwj (if you are the one I should ask, you were the most active user in this repo) If a really good pull request that adds this to the json output is submitted, will it be accepted?
