I upgraded to the v0.9 64bit mac today. When sending a transaction of .09, I decided not to pay the fee of .0002, clicked cancel, and it sent the transaction anyways.
This was the third or fourth time sending and canceling to the same address.

Are you sure you didn't just click the wrong button?
As confident as one can be. I clicked, wrong action happened, profanity ensued.

Reply to this email directly or view it on GitHubhttps://github.com/bitcoin/bitcoin/issues/3913#issuecomment-38140445 .
If this can be reproduced, this it is a big problem. @patrickmason, try doing that same thing again (offline!) and then rollback your wallet afterwards. A video would be nice, plus any debug logs.
Can someone reproduce this? Or any other updates about this?
I have not been able to reproduce it, must have been human error.

I've also never been able to reproduce it. Without further evidence (or reports from other users) I'm going to close this.
